Life Skills Program:

It is recommended that the Applicant be registered with Mental Health & Addiction Services to ensure adequate case management and to meet admission criteria.

Supported Independent Community Living Program:
It is recommended that the Applicant be registered with Mental Health & Addiction Services to ensure adequate case management and to meet admission criteria.

Day Program:
It is recommended that the Application be completed by the Applicant's General Physician, Psychiatrist, Community Mental Health Nurse or Interagency.

Upon receiving an 'Application for Services' for ANY of our Programs,
an assessment/review will be administered by Libbie Young Centre’s Admission & Discharge Committee to determine the appropriateness of the Applicant.
